In order to make a new patient appointment, we need a referral from a physician provider unless you have none or this is a family member. Once we receive referral paperwork, we can schedule an appointment and send out by secure text a new patient electronic forms.
If you are already a patient and would like to make an appointment, please call us at 615-791-5470 to schedule it or find us at Follow My Health and add us. We will text you paperwork my secure messaging; complete it before your appointment.

If you cannot navigate these we have forms. Please only use the forms if you cannot use electronic documents.
Be sure to bring copies of your ID, current insurance cards and any pharmacy plans.
